CASE STUDY OBJECTIVES
After completing this case, users will be able to:
- Describe hypertension and its associated risks and risk factors
- Undertake nutrition assessment on hypertensive individuals by collecting and interpreting nutrition assessment data specific to the development and treatment of hypertension including food/nutrition-related history, anthropometric measurements, biochemical data, nutrition-focused physical findings, social and medical history
- Practice diet analysis using the ready reckoner approach
- Critically assess and cluster nutrition assessment data to prioritise nutrition issues of concern, select Nutrition Diagnoses and create PES Statements for an individual with hypertension
- Integrate previous Nutrition Diagnoses with new Diagnoses
- Apply knowledge of food, nutrition, physical activity, health and disease to develop a client-centred nutrition prescription and interventions for hypertension
- Develop a therapeutic diet for weight management including appropriate nutrient sources and meal plans
- Develop a Monitoring and Evaluation Plan with nutrition outcome measures and performance indicators for a patient with hypertension and obesity
- Document the nutrition care process using accepted standards
- Explain the diet-disease relationship
